Northern Counties Health Care

Northern Counties Health Care is a federally qualified health center system serving Vermont’s Northeast Kingdom. It operates a rural network of primary care clinics and dental centers, and it also provides home health and hospice services. Patients can obtain preventive and chronic disease care, behavioral health support, physical therapy and care coordination through the system. Its administrative office is in St. Johnsbury. The organization works with area hospitals and community partners to connect residents with services across a largely rural region.
